I'm using GFI Network Server Monitor (http://www.gfi.com/nsm/) to monitor my servers here at home as well as my backup mail and dns servers at a friend's place. It's easy to use and has a number of preconfigured monitoring programs for SQL, Exchange, ISA etc. In case of problems, it sends out an alert by e-mail, net send, or sms.
